Just from experience and being around 'the scene' for a decade or more, one thing to consider is the tolerance issue with oxycodone, especially oxycontin users. I've known many folks who could maintain a reasonably stable level of med intake to control the pain with most pain meds, with the occasional increase in dosage needed of course, but i've seen far too many people get onto the OC train and go from 40 mg a day to 160mg a day in just a couple months or less. I have no statistics to prove it but knowing a lot of pain patients who have been on the gamut of drugs, all seem to agree that oxycontin was the one that seemed like the tolerance would never stop going up. YMMV of course, but out of dozens of people I talk with, I can't think of any that would disagree with what I said.
Personally I think methadone is a great long-term pain reliever because you only have to dose once a day for the most part, and your tolerance doesn't seem to climb very fast at all unless you're abusing it or doing something else wrong...

ETA - regarding the IR oxycodone and tylenol, true roxicet has it but roxicodone is apap free...
