Mark, list the medications you've been prescribed & are taking when you give history information when they fill out the paperwork for your screen...If they want further information after the screen they will ask for it...it is highly unusual for them to ask for documentation of meds you list as prescribed for a medical condition...DON'T LIE, and DON'T take any meds *(for example from IOP's) that you don't have a VALID US PRESCRIPTION FOR...US OP's scripted from a LICENSED PHYSICIAN should be fine....don't OFFER the fact that you are getting the medication over the internet...if and it's a big IF they want further documenation...give them the name & phone number of the PHysician prescribing & let them call him...
