Here's how to order your consultation and prescriptions:

1. Read and agree to our Terms of Use. 

2. Register and complete the medical history questionnaire. You will provide us with the information your doctor will need to consult with you regarding your medical condition. Upon submitting the questionnaire to us, you will be prompted to attest that all of the information submitted is true and accurate.

3. Enter your payment information.  Your credit card will only be charged if we are able to arrange a physician's phone consultation that results in a prescription or diagnosis. However, a $30 administrative fee will be charged following a physician consultation that does not result in a prescription being written.

4. You will receive an e-mail message confirming your phone consultation appointment with a time for the physician to call you. Please make sure we have a valid e-mail address.  Most of our problems communicating with our patients results from incorrect e-mail addresses.  You will be assigned a time that has been scheduled specifically according to your request. We are committed to providing you with a simple method to consult with our physicians, and we request that you are prompt with your telephone appointments.

5. Please fax a legible copy of your driver's license, U.S. Passport or Military ID and medical records to (309) 413-4827 prior to your physician consultation time. Our affiliated pharmacy will not ship any prescriptions without a legible copy of your medical records or identification.

6. DoctorByPhone.com is a customer service agency, not a pharmacy.

7. Your physician will call you at your scheduled appointment time.  Please be prompt.  Your physician has the ultimate authority to diagnose medical conditions and/or offer a treatment option.

8. Upon a successful diagnosis by your physician, we will:

   • Charge your credit card in the amount of $110 for your physician consultation fee

   • Forward any prescriptions from the physician to our participating pharmacy

   • Ship the prescription to you

9. The pharmacy will separately charge your credit card for the cost of your prescription drugs plus shipping and handling

10. United Parcel Service (UPS) will not deliver to PO boxes. UPS requires a signature on delivery from a person 18 years of age or older

11. Once you have opened your package, please read the enclosed prescription information sheets that are included with each prescription. The enclosed medication has been specifically prescribed based on the information conveyed during the physician consultation, and from your medical history. DoctorByPhone.com and our physicians do not assume any risk that results from the use, misuse or abuse of any medicine prescribed by our physicians. 12. No charges will be made to your credit card without a successful diagnosis by a physician.

13. Pursuant to Federal and state law, we are not permitted to issue refunds or exchanges of prescription medications.  All sales are final.
