How does someone go about getting a medical history record (report)? My records are in another city - it's been years. I have migranes and trying to deal with new doctors is why I am wanting to do the OP thing. Any info is greatly appreciated.

write to your old docs or call them. they will probably charge you about $50 for copies. Most of them will transfer records to another doc for free, but you have to sign a waiver with both docs. If your old doc is really cool (few are, all should be) he should be willing to email or fax your records to the OP free of charge. good luck.

You should probably call your doctor's office and request that they send them to you. Some offices charge a fee. In the case of my doctor, he would only send my records to the doctor's office, so I had to sign a release form from the op to the doctor. That process took about 3 weeks in my case.

The doctor's office was probably not following the state law if they refused to give you your medical records. Until recently it was only state law that governed, but now the federal HIPAA grants every patient the right to get copies of their medical records. If they refuse you now, they can be hit with a stiff federal fine. They can charge you a fee, but they have to give the records directly to you if you request them and sign a release.

Just a thought, but if your records are over a year or two old, you might consider going to a minor medical "walk-in" clinic and get "treated" for the conditions that are bothering you. Be sure to get a copy of the office visit and the doctor's notes. You may consider setting up a new PCP visit if you have insurance or money is not a problem. I say this because most OP's will only accept records that are 1 year old or newer.
