Just a thought, but if your records are over a year or two old, you might consider going to a minor medical "walk-in" clinic and get "treated" for the conditions that are bothering you. Be sure to get a copy of the office visit and the doctor's notes. You may consider setting up a new PCP visit if you have insurance or money is not a problem. I say this because most OP's will only accept records that are 1 year old or newer.
