Can a government agency with no reason just look at your medical records even if you gave 'permission' via a notary signed form?
You haven't said what type of permit it is, but if it's a concealed carry weapon permit, then can ask you to sign a release and deny the permit if you don't sign the release. My guess is that if you deny them access and appeal their decision to not grant the permit, they will go to court to say that they *do* have a reason. All they need to do is show a rational basis for the request and be compliant with HIPAA.
If you want a better answer, tell us what kind of permit this is. HIPAA limits the requests that government agencies can make for your records but there are a lot of exceptions. They also need a rational basis. A permit is generally a privilege, not a right.
