I am done with setting up consults for Monday, April 26th so if you did NOT get an e-mail pleaseee e-mail me and we will figure out why you did not receive an appt. I don't know your real name ( since we go by login names here) so, please let us know via e-mail if you received your appt. IF you did NOT, then it could be one of 2 things:
1), we did not receive your fax or
2), your fax was not complete ( if you are a new patient we need the medical questionnaire, ( you log into out site and click on "request a consult fo rnew patients and print that out, fill it out and fax it back to us with your medical records, and photo ID>) The medical questionnaire is VERY important for new patients so that we have a very well documented medical information package for all of our patients. It not only protects us but it also protects you medically so that we have a complete history for all of our patients!!

Thanks for your call today (Sunday).
Since then, I registered on YOD's web site, faxed over all my medical records, the Y.O.D. completed forms and copy of my current license.
I do not expect it, but I would greatly appreciate a consult Monday 4-26 at YOD's earliest convenience.
