if you go to www.rxhope.com and register with them, you can download the forms yourself and be one step ahead. I did this for my Effexor and Depakote when both me and my husband were out of work. I got them, but it took a while. The Depakote was the wrong kind, so be sure what your doctor fills out on the form as for what you need is right.

Good luck! The forms vary from company to company as does the documentation. The documentation for the drugs I got was minimal.
