if you go to www.rxhope.com and register with them, you can download the forms yourself and be one step ahead. I did this for my Effexor and Depakote when both me and my husband were out of work. I got them, but it took a while. The Depakote was the wrong kind, so be sure what your doctor fills out on the form as for what you need is right.
