Ultram and Amitriptyline-seizure problems?
      #121317 - 12/08/03 06:09 PM

I was recently prescribed some amitriptyline for help with sleep and for back pain. I wonder if there is any chance that there could be an increase and problem with my seizure threshold being on the two drugs?

Well, I can tell you this. My neuro has done an EEG and found me to be prone to seizures. this after having one last year attributed to Wellbutrin. He was going to give me some amitriptyline to help with sleep, but gave me Midrin to help with Migraines instead. I don't know if that type of thing is useful to you or not. He wouldn't prescribe both because he said he won't give out 2 scripts on one visit. (He's such a lovely doctor - feel the sarcasm)

For what it's worth, I've taken both at the same time and had no problems.

But I would talk to your doctor.

This is pasted from drugs.com :

"...What other drugs will affect Ultram?
• Ultram may increase the risk of seizures especially in patients who have epilepsy or another seizure disorder. Also, Ultram may increase the risk of seizures if you are taking any of the following drugs:
· a tricyclic antidepressant such as amitriptyline (Elavil), nortriptyline (Pamelor), doxepin (Sinequan), imipramine (Tofranil), clomipramine (Anafranil), and others;
· a monoamine oxidase inhibitor (MAOI) such as isocarboxazid (Marplan), phenelzine (Nardil), or tranylcypromine (Parnate);
· an antipsychotic medication such as chlorpromazine (Thorazine), fluphenazine (Prolixin), haloperidol (Haldol), loxapine (Loxitane), mesoridazine (Serentil), perphenazine (Trilafon), thioridazine (Mellaril), thiothixene (Navane), and others;
· a selective serotonin reuptake inhibitor (SSRI) such as fluoxetine (Prozac), fluvoxamine (Luvox), paroxetine (Paxil), sertraline (Zoloft), or citalopram (Celexa);
· a narcotic pain reliever such as codeine, fentanyl (Duragesic), hydromorphone (Dilaudid), meperidine (Demerol), hydrocodone (Vicodin, Lorcet, Lortab, others), morphine (MS Contin, MSIR, RMS, Roxanol, others), oxycodone (Roxicodone, Percocet, Percodan, others), propoxyphene (Darvon, Darvocet, others), and others;
· promethazine (Phenergan) or prochlorperazine (Compazine);
· bupropion (Wellbutrin, Zyban); or
· cyclobenzaprine (Flexeril).
• Do not take Ultram without first talking to your doctor if you are taking any of the medicines listed above..."

This is from WebMD:

"...Seizures have been reported as a rare side effect of treatment with tramadol. The risk of seizures may be increased in patients who have any of the conditions or are taking any of the medications listed below: Do not take tramadol without first talking to your doctor if you
· have a history of seizures or epilepsy;
· have a head injury;
· have a metabolic disorder;
· have a central nervous system infection;
· are experiencing alcohol or drug withdrawal;
· are taking a tricyclic antidepressant such as amitriptyline (Elavil), nortriptyline (Pamelor), doxepin (Sinequan), imipramine (Tofranil), clomipramine (Anafranil), and others;
· are taking a monoamine oxidase inhibitor (MAOI) such as isocarboxazid (Marplan), phenelzine (Nardil), or tranylcypromine (Parnate);
· are taking a psychiatric medication such as chlorpromazine (Thorazine), fluphenazine (Prolixin), haloperidol (Haldol), loxapine (Loxitane), mesoridazine (Serentil), perphenazine (Trilafon), thioridazine (Mellaril), thiothixene (Navane), and others;
· are taking a selective serotonin reuptake inhibitor (SSRI) such as fluoxetine (Prozac, Sarafem), fluvoxamine (Luvox), paroxetine (Paxil), sertraline (Zoloft), or citalopram (Celexa);
· are taking a narcotic pain reliever such as codeine, fentanyl (Duragesic), hydromorphone (Dilaudid), meperidine (Demerol), hydrocodone (Vicodin, Lorcet, Lortab, others), morphine (MS Contin, MSIR, RMS, Roxanol, others), oxycodone (Roxicodone, Percocet, Percodan, others), propoxyphene (Darvon, Darvocet, others), and others;
· are taking promethazine (Phenergan) or prochlorperazine (Compazine);
· are taking sibutramine (Meridia);
· are taking bupropion (Wellbutrin, Zyban); or
· are taking cyclobenzaprine (Flexeril)..."

I'd be cautious.

I saw a new doc yesterday because my old doc was a worthless piece of you know what. I would have to practically beg him to get a prescription any time I needed one. This new doc wrote me four prescriptions right on the spot. Hardly any questions. I have a feeling that I am going to like him. Maybe I can finally get some of the relief that I need!

Well, well, Mr. Pinhead I'm happy for you! Just be careful of the seizure thing - they aren't much fun. After you have one, you always wonder. I still jolt around (like touching an electric fence) a little, and that worries me. Just watch your body, the day I had my seizure, I felt weird, and had tons and tons of "twitching" and then "BAM" I was on the floor looking up at EMS. Anyway, not trying to scare you, just concerned.

I love my Pinhead, ya know??

What dosage of the tramadol are you taking daily? (be honest here)....any dose equal to or greater than 400 mg can cause grand mal seizures & the amitry. has the unfortunate side effect of prolonging the half life of almost ALL other meds...so you may need to LOWER your dose of the ultram. and if you've had a seizure, stopping or lowering the dosage won't necessarily stop the seizures...I continued to have seizures after discontinuing ultram altogether for 5 years....on seizure meds....think about it...you may be driving along one day and have a grand mal & kill you, or worse yet the family of five in the car next to you....you need to see a neurologist
