Someone correct me if I am wrong but I beleive that is a sort of medical form universal to the medical field. That way they know you did not just create it on your computer.

It's the yellow copy of your visit summary they give you when you leave the office. It notes your diagnostic history as well as the diagnoses of the current visit, your insurer, co-pay, etc.

Thanks for the info, so I should just be able to go to the Dr. that ordered my xray and prescibed the celebrex to get a superbill. Can I get a superbill from an OP ive been using?

No.... Superbills are the receipts you get (that include procedures performed/diagnostics) from an actual office visit.

When I went to my DO, the auto insurance took care of my bill, so I never recieved one of those slips. I have before, but not for my MVA.

This makes me think of credit card receipts. The yellow copy is the superbill. So , are all superbills the same??? Is there a specific form??? Cause if you just needed a yellow copy of a medical bill, you could make it up and tear out the yellow copy and use it. ( saying you had the 3 part paper ) not suggesting in anyway to do this at all!!! Just wondering if its to keep people from printing their own records?

A superbill is a two columned long list of procedures and diagnostics. At the bottom right corner there is usually 4 or so blank spaces where the physician scripled something like initials in each one. At the top of the bill is your name, the doctors name and the reason for the visit.
Unless you have some medical experience you cannot forge these things. And, yes, you should get one after every visit. This is not the same as medical records which give details such as height, weight, blood pressure, history of visits and diagnosis with medication prescribed.
The Superbill is what the insurance company gets to accurately assess and verify the visit costs. You should get one even if you are uninsured.
Go to the Doc's office and ask for a copy of your records and include the superbill copies, also.
Hope this helps.

Talked to my DO. He doesn't use a superbill, but rather a billing service in another city for insurance claims.
Just FYI
