For same day consults You must put SAME DAY CONSULT REQUEST on your cover sheet or on the shipping and billing form. However, please do not always depend on this as we are very busy at times and cannot always do same days. Please make sure to fax your request for a same day consult ( must be a complete fax) over to us as early as possible as some are faxing after 3:00 especially on Fridays and we cannot get them in as we are not going to be able to get all processed before 4:00 when we close on fridays for the weekend.

I just wanted to put a quick post to let everyone know that all you need to so is this for second consults:
1) Print out the "shipping and billing" form from the 8 page download only.
2) Write on the top of the form "second consult"
3) Fax it to: 603-375-3099
That is all you need to do.
