I think that you should refill. You should always have a little emergency medicine in the house in case something like this happens! It's like an insurance policy because when the horrible toothache happens, it is always on a weekend and if you call the dentist, he'll act like you are a drug seeker and make you feel bad.
I'm sorry that you are feeling so poorly. I would definitely go to the ER and explain what happened or if you have a regular doctor (if he is a nice doctor), call him and ask him if he could prescribe a few days worth of pain meds until you can get in to see him. And definitely get your refill.
I'm so sorry to hear about your accident, and hope you feel better and pain free soon.
