BOARD OF PHARMACY LICENSING STATISTICS
The Board currently licenses/registers a total of 20,435 individuals and facilities. The following
provides a breakdown for each category of license/registration:

RULE UPDATE
The following rules were amended by the Board. 4CSR 220-2.200 Sterile Pharmaceuticals will
go into effect on July 1, 2004. Rule was replaced in its entirety and places new requirements on
pharmacists compounding such products. 4CSR 220-2.400 Compounding Standards of Practice
went into effect on July 31, 2003. This rule was amended to add in various provisions dealing
with quality assurance and updating minimum standards. Both rules can be accessed through the
board’s web site at www.ecodev.state.mo.us/pr/pharmacy and clicking on the icon for rules and
regulations. Once on the Secretary of State site, click on Division 220 and then click Chapter 2.
GOLD CERTIFICATES
The following pharmacists received gold certificates in honor of maintaining a license with the
Board for 50 years. Each gold certificate is signed by the Board members, the executive director
and Governor Bob Holden and is accompanied with a letter of congratulations from the Board.
Congratulations to those who have served the public for 50 years as a licensed pharmacist.

Lloyd G. Bottoms, #42116, Nixa, MO--May 29, 2003. Probation for one year, until May 28,
2004. Falsified prescription records to establish eligibility in a class action lawsuit. Section
338.055.2(5), (13), and (15), RSMo 2000.
Robert R. Courtney, #29228, Kansas City, MO--May 1, 2003. Revoked, cannot reapply for
licensure for seven (7) years. 3/01 through 6/01 adulterated and delivered 103 prescriptions for
Gemzar and 57 prescriptions for Taxol. On August 7 and 13, 2001, diluted 2 prescriptions for
Gemzar and 1 prescription for Taxol with another substance, or substituted another substance for
Gemzar and Taxol. Tampered with and diluted prescriptions for Platinol and Paraplatin.
Submitted false claims to Medicare for all of these prescriptions. 2/26/02 Courtney pled guilty to
8 counts of Class C felony tampering with a consumer product, 6 counts of Class E felony
adulteration of a drug, and 6 counts of Class E felony misbranding of a drug. Section
338.055.2(2), (4),(5), (6), (13), (15), and (16), RSMo 2000.
Daniel B. Drewry, #41934, Springfield, MO--May 29, 2003. Probation for two years, until May
28, 2005. Numerous dispensing errors, unauthorized refills. Section 338.055.2(5), (6), and (13),
RSMo 2000.

Michael A. Hefley, #29099, Blue Springs, MO--March 24, 2003. Probation for three years, until
3/23/06. Violation of previous disciplinary action; audit of controlled substances failed to
account for loss of drugs and failed to timely notify officials of losses. Section 338.055.2(5),
(13), and (15), RSMo 2000.
James E. King, Jr., #44618, Kennett, MO--June 14, 2003. Probation for three years, until June
13, 2006. Diverted Tramadol from employer for personal use. Section 338.055.2(5), (13), and
(15), RSMo Supp. 2002.
Blain L. Larson, #2003005974, Fairbury, NE--March 14, 2003. Restricted license issued on
Probation for five years effective when lives in Missouri. Disciplinary action in Nebraska
regarding unauthorized dispensing without a valid prescription. Section 338.055.1 and .2(5), (8),
and (15), RSMo 2000.
Gary S. Ravis, #27656, Leawood, KS--May 21, 2003. Revoked, cannot reapply for licensure for
three (3) years. Knowingly received, possessed, concealed, stored, bartered, sold and disposed of
stolen prescription drugs and medications, valuing $5,000 or more, which crossed a state
boundary after being stolen. 12/01 pled guilty in U.S. district court to Class C felony for conduct
described above. Section 338.055.2(2), (5), and (13), RSMo Supp. 2001.
John S. Vogel, #30074, Chesterfield, MO--April 30, 2003. Probation for five years, effective
4/30/03. Violation of previous disciplinary action; failed to keep Board advised of employment
address, failed to keep perpetual controlled substance inventory, failed to obtain new
alcohol/drug counselor/program and did not insure counselor reports submitted to Board as
required and failed to submit 6-month progress reports.

Jill D. Williams, #2001007711, Newburgh, IN--March 8, 2003. Probation for three years,
effective 3/8/03. Falsified controlled and non-controlled substance prescriptions for personal
use. Section 338.055.2(5), (6), (13), (15), and (17), RSMo Supp. 2001.
PHARMACIES:
Lee Pharmacy, Inc., #2000154854, Fort Smith, AR--March 6, 2003. Suspension for three
months effective 3/6/03, followed by probation for five years. Sale of compounded drug
products without patient-specific prescriptions, unlicensed drug distributor activity. Section
338.055.2(5), (6), (13), and (15), RSMo Supp. 2001.
Marrs Pharmacy, #5804, Blue Springs, MO--March 24, 2003. Probation for three years, until
3/23/06. Violation of previous disciplinary action; audit of controlled substances failed to
account for loss of drugs, failed to timely notify officials of losses. Section 338.055.2(5), (13),
and (15), RSMo 2000.
TEMPORARY RESTRAINING ORDERS/INJUNCTIONS:
Med 4 Home Pharmacy, #2001030511, Kansas City, MO--Temporary Restraining Order issued
3/10/03 by Platte County Circuit Court, Final Judgment Granting Permanent Injunction Pursuant
to Consent Agreement Between the Parties filed 5/5/03. Action taken due to unknown sources of
contamination of compounded products, failure to inform prescribing physicians and patients that
medications could be contaminated, failure to provide all records relating to the contamination of
products and any corrective measures taken and denied board inspectors full access to the
pharmacy.
