That "disclaimer" / "warning" is located on more than one nrop site ... maybe even all of them. I don't know about a national database; but there have been threads here regarding a national database of pharmacies, and the gist seems to be that the databases are on a state level only. I can tell you that I don't recall ever seeing a post from someone who said they were denied their order from an OP because their name came up on a database check.
I have, however, read a post recently where a member went to their doctor and the doctor had found out about their online ordering. You may want to type in "national database" in the search section to read previous posts regarding this issue.
