Personally,

I would never, ever, create and sign a statement like the one you mentioned. I can sympathize with your anger towards the individual who used your name for his own gain. However, I would never take it to the point of naming who "he told you he sold to, and who you think bought from him".
To me, that makes this situation different and changes your role in this persons case (which is completely his fault).
This is just my opinion.
