The main point of this rule is that a medical questionaire is not sufficient for the practice to be legal. There must be valid medical records, and physicals, and they both must support the diagnosis that the meds are being prescribed for.
Further, the medical records must be kept up to date, and at every consult, the prescriber must discuss the risks and benefits of the prescribed therapy, as well as discussing alternatives to that therapy. Every bit of this has to be documented, in a patient record, maintained by the OP.
Porta-Medic exams, while called a physical, will probably not meet the requirements of this rule. It will be necessary for the patient to have a "real" physical, by a licensed physician, with bloodwork and diagnostic imaging, to meet these criteria.
