What you need is two things (which can be accomplished in only one office visit if you tell the doc what you want) - A recent physical (within one year) - to sort of show that you are in ok health as far as blood pressure, etc... And a diagnosis of the medical condition you are seeking relief from. What I did (since I have a long-term condition of my spine, that does not require frequent doctor visits - just have to 'live with it' basically according to my family doc) - What I did was went in, and told him I was going to see either an Orthopedic or a Pain-Specialist, and before I went, I needed a good document of current physical, and re-affirmation/diagnoisis of my spinal condition. I told him before the examination that I was doing this for records that I needed. He was fine with that, although a bit disappointed that I wanted to seek help 'elsewhere'..... Anyway so I got a full, good physical and re-diagnosis (x-rays) of my spine, and this is all documented on a couple pages, and has worked fine for me. Best thing to do if you don't have current medical records, is visit the doc and tell them *up front* that you need a good physical and re-diagnosis of your condition for your records, or as documentation to get in to see a specialist.
