A superbill is a two columned long list of procedures and diagnostics. At the bottom right corner there is usually 4 or so blank spaces where the physician scripled something like initials in each one. At the top of the bill is your name, the doctors name and the reason for the visit.
Unless you have some medical experience you cannot forge these things. And, yes, you should get one after every visit. This is not the same as medical records which give details such as height, weight, blood pressure, history of visits and diagnosis with medication prescribed.
The Superbill is what the insurance company gets to accurately assess and verify the visit costs. You should get one even if you are uninsured.
Go to the Doc's office and ask for a copy of your records and include the superbill copies, also.
