Contugesic - Dihydrocodeine (DHC)
      #156299 - 04/20/04 07:07 AM

Dihydrocodeine (DHC) is a synthetic opioid analgesic developed in the early 1900s. Its structure and pharmacokinetics are similar to that of codeine and it is used for the treatment of postoperative pain, severe breathlessness or as an antitussive.

Dihydrocodeine is approximately twice as potent as codeine; take this into consideration while dosing dihydrocodeine.

has anyone sampled the dihydrocodeine descibed as "codesic" by an asian pharmacy or the "df-118s" offered by another asian pharmacy?
do any DHC experts reckon they are purely DHC or just codiene sulphate or even worse-a counterfeit product with not the true 30mgs dose?

I've had the Codesic a few times. I believe it's the real thing, same as Didor Continus or Contugesic except it's not the sustained release.

Yes, two 30 mg tablets seems the same as 1 chewed Didor Continus or Contugesic

What else is in the 30mg from "that" place? They weigh 300mg. What is the other 270mgs?

IT could be how they are disposing of the nuclear waste. I would test for radioactivity!! Actually it is what would be called an inert filler (usually plutonium).

you can not base stregth on weight. ie oc 80 mg they do not weigh 80 mgs.

Question for anyone who has taken DHC. I know it's like codiene, but how "Like" it is it? I ask, because I can take Hydro and Oxy and everything with no problem, but Codiene makes me itch like you wouldn't believe. I've never tried DHC, nor do I plan on it, but I was wondering in case I was ever in a situation that I found I had no choice but the DHC if I should take it or if it'd do the same thing to me as codeine. BTW, just a little tid bit of fun, you know the I before E rule, except after C and ....., well Codeine is one of a few limited words that doesn't follow that rule:)

If codeine makes you itch, DHC will do the same thing. All you have to do is take a antihistamine to stop the itch. I take one Benadryl and it stops any itch. Without it, the itching is unbearable.

codeine never makes me itch even at quite high dosage but DHC makes me scratch myself to shreds at minimum dosages. I need to take 25mg phenergan 3 times per day when taking DHC & even that doesnt prevent all the itchies; it just makes it a bit easier to avoid scratching myself till I bleed. It's the one side effect I cant stand.

I tried DHC a couple of times. It was terrible. It was a lot like withdrawl. It was similar to the time I was accidentally ( through ignorance) abusing oxycontin. I was very agititated and just felt awful. No other opiates do that to me. The pills are tiny white ovals, imprinted "DHC 60". Are these time release?? Any other thoughts? It would be nice to have these for a backup, but I don't even want to experiment again unless I have an understanding of what happened, and I am confident I have a way to manage things.

Contugesic isn't made anymore, it's been out of production since 2003. The only med that contains 60mg of time-released DHC now is Didor Continuus.

I would like to correct Toe's statement about the production of Dihydrocodeine.
In addition to the time released 60mg. tablets of Dihydrocodeine produced by Viatris in Spain, labeled as Didor Continus.
It is also available and being produced in South Africa by GlaxoSmithKline in 30mg. instant release tablets labeled as DF118.

Re: Contugesic - Dihydrocodeine (DHC) [Re: curve]

Didor Continus is used to treat moderate to severe pain. Here are some experiences from people I found on a website. If anyone knows of a source please pass it on.

- On a recent trip to South Africa, I tried to lift a heavy suitcase and collapsed. The hotel doctor was summoned and prescribed 60MG of Dihydrocodeine. I can state without equivocation that this drug is extremely powerful; within a half hour of taking 60 mg, I was totally free of pain and felt a sense of mild euphoria. I normaly take OxyContin 60MG twice daily. As everybody knows, OxyContin is a very powerful drug. I have found that Dihydrocodeine works just as well as OxyContin. This drug has satisfied my pain relief needs on those occasions that I have not been able to obtain OxyContin.

- It works pretty well and is kinda like hydro. It works great.

- Basically, dihydrocodeine is comparable to our hydrocodone or vicodin except that it contains no tylenol, which is great for those of us who need the medication in higher doses. Save your livers, try to order the dihydrocodeine. It is also about 2.5 times more potent than regular codeine phosphate because its molecular structure allows the drug to be converted quicker in the brain tomorphine and does not completely get absorbed in the liver. Anyway, dihydrocodeine is good stuff, but alot stronger than regular codeine.

- On a personal level, I found Dihydrocodeine a great help in managing my sciatic pain. I took it for about eight weeks, until I was admitted to hospital where a wonderful orthopedic surgeon put my back right for me.

- In my opinion this is a fantastic painkiller with few side effects, the worst of which can be avoided by a good doctor. As for the constipation, there is a little secret cure, and it's good old fruit and vege, and lots of it! A little Aloevera (a medicinal cacti used for the last 4000yrs) extract also does wonders, you can by it in pill or juice form and can find it in your local health food store.

- I just started taking Dihydrocodeine about two months ago to treat chronic neck pain. Vicodin was not working, so I guess this is the next step up. I only use it when the pain gets really bad, which is every couple of days or so. As far as addiction goes, I would have to say that it is not addictive, as I can take it for two days then go off it for three or four days with no adverse effects. I haven't developed a tolerance to it at all either. I like this drug because it eliminates my pain, but doesn't make me totally non-functional like Vicodin does. You need to stay hydrated, so drink plenty of water or Gatorade. The other thing to watch is constipation.

- Dihydrocodeine is not 2nd only to morphine in potency. Two other opiates, Oxycodone and Hydrocodone, are approximately three times as potent as Dihydrocodeine. Oxycodone is used in Oxycontin and in Percocet, while Hydrocodone is used in a myriad of products - most common of them being Vicodin and Lortab. Dihydrocodeine is an excellent drug, especially in that it is not as addictive as these others, but as one ""goes up the ladder"" one would use Hydrocodone (after Dihydrocodeine), then Oxycodone, and then morphine. Note that there are others even more potent than morphine. For example,Hydromorphone is approximately 10 times as potent as morphine.

Ok, please check my math. Something doesn't seem right.

mg/mg, hydro is 6 times as potent as codeine (phosphate). DHC is twice as potent as codeine. Therefore, hydro is 3 times as potent as DHC. If I am taking 40mg of hydro per day and want to change to DHC, I would have to take 120mg per day. I have the "continuous" 60mg DHC pills, so I would take 2 of these per day to get the same effect as 4 10mg hydros per day. Right?

I don't know how to tell you to calculate the dosage for type of dhc you are taking JethroBodine as I have never took it .I have been getting the 30mg codesic [dhc] made in Malaysia .To me they are almost as powerful as 10mg hydro not quite the same effect but good .They are only $1.25 a piece from the IOP I have been getting them from .It beats ordering from the domestic NROP,s that charge $3 to $4 a piece for the 10mg hydro and seem to go out of business every other week now .I prefer the domestic hydro but have not been able to get a order since August.I think I will just keep getting the dhc until I come up with a better alternative.I guess my post is slightly off topic but that is my opinion .

I have tried both Didor Continus 60mg. and Codesic 30mg. I have yet to try DF118. In my opinion, 60mg of DHC is comparable to 5mg. Hydrocodone. But I would like to add that DHC seems to stay in your system much longer than Hydro. All I hear is good things about Dihydrocodeine, and how effective it is for pain relief. I have yet to hear one negative thing about it. The thing I would like to know is why is it not prescribed more often, and made available in more than the just a few low dosage medications. Could someone tell me the names of the domestic versions of DHC medications available, and what the different stengths are?

I am answering my own question. After doing a little searching I found three different medications containing Dihydrocodeine available locally. They are as Synalgos-DC, Panlor, and DHC plus. They all contain 16mg. Dihydrocodeine, 356.4mg. Aspirin and 30mg. Caffeine. I do not understand why they are not available in a higher DHC count.

Re: Contugesic - Dihydrocodeine (DHC) Liver damage? [Re: curve]
      #220279 - 01/30/05 04:24 PM

back to the topic to what else is in those 30mg DHC tabs. I had been taking 6 per day for a couple of months & got a routine liver function test that showed almost all levels at 3 or 4 times normal range. My doc said its not unusual to see levels like that in response to some medications. I am on other meds that Ive been on for years without anything like this showing up. The only new med in the mix was the dhc.

Im making the assumption that this med at that dose for that long was battering my liver. I was scared sh|tless when I got those results & I dont know what to do.
Are the Didor Continuus available from an IOP? If so please reply or pm me which one it is. Im going to save the 30mg tabs for emergency only use from now on.

Rara: Could you please tell us where the DHC's you are talking about are from ?

DHC like codeine, should in a short while break down to morphine in the blood and little more that matters. Morphine in the blood is definately addicting, although if you keep doses low, problems arise far less so. I remember someone before saying they thought something is being added to the DHC from the Phillipines. I am not convinced that there is anything added to the those pills. DHC has subjective effects that are different from codeine which some people prefer and others do not. Supposedly DHC is less foggy and less sedative than regular codeine, so that description of DHC sort of describes hydro to me. Of course metabolisms differ and people respond differently toevery medication. Some people say DHCs strength seems on par with codeine others say 1.5 times stronger and above I read twice as strong. I think the difference in perspective depends on what ails you. If someone measures their medicine by how happy they become from the drug, then a very different viewpoint will be held than from someone who measures or compares a medicine's pain killing relief. Some physical body ailments might be better served by one over the other, so the strength would vary by condition.

If someone has a lot of experience with true, well labelled DHC from the US or Europe, and they have also used the unlabeled, no brand Philippine variety, I'd like to know how do they compare?

If you are comparing the Liver damage of DHC with Hydrocodone, I may be wrong, as I am no physician, but usually the liver dammage associated with Hydrocodone is from the APAP that is in them (Acetaminophen=APAP)
example: 10/500 = 10mg Hydrocodone & 500mg APAP. Or 10/325 = 10mg Hydrocodone & 325 APAP. I am aware that most of you guys know this, but I am sure that a few of you don't. I hope I have helped at least one person, that would make it worth the time I have spent. Thanks for a great informative forum. You guys have saved me hours of research at times. Just trying to give a little back

I really miss Contugesic. I routinely substituted it for the hydrocodone I was taking, to give my liver a break from all that APAP. While it wasn't as good as hydro when it came to giving me energy and a sense of well-being, it was really almost as effective for my pain. But I think what I liked the most was the extended-relief factor. It would easily last 6-8 hours.

It's my understanding that Didor Continuus is the same thing. Am I correct? Is there a source for this? If someone would post that information, I'd really appreciate it. If you'd prefer, send me a PM.

TIA

Didor Continuus is another brand name for 60mg time-released DHC, yes. The reason I posted is because brand name Contugesic isn't being made. You can still get DHC.
