I had suspected this about 2 summers ago when we had some hard feelings about an issue that I had no control over( our rleationship has not really improved since then, but I stay with this guy for the next 1 and a half years for a reason that is unrelated to what you might think)
I suspected he wrote some negative things about me or the fact what he wanted to give me did not work and it mad him him mad or perhaps I did not respond to his treatment at the time. These treatments run from getting it right (rare) to being able to live with it and he is the best I have for the time being (most common).
Another time I KNOW the record was changed because I mentioned that I would like to go back to a certain drug because the other two he had tried did not work.
He flipped thorugh his records of me and said that I had told him that I had trouble breathing with this drug.
Wrong. I said that it did not work at the dose he prescribed and maybe it would work at a higher dose .
A couple of months ago, I wanted to update my copies of the medical records in his file on me since I was missing those records from the summer 2 years ago that we started having problems and wanted to see what he did actually write.
The result. There was no mention of any friction which I figured would not be in there since doctors like everyone else CYA and again it was just my supicious nature.
But when I tried to find the visit he wrote that I had trobule breathig on the mentioned medication, it was not there!
I know that doctors practice crappy handwriting in med school I think like lawyers use latin phrases. Make an everyday question hard to decipher or whatever. But I can usually read a doctor's instructions sue to seeing a lot over the years so I kind of broke the code though I miss a few things still.
But if I HAD an allergic reaction to a medication, it is his medical and legal duty to note it.
I know that doctors are so concerned over lawsuites (though I rarely see any but then I don't look for them in the news) but I might go to another doctor and ask to try the drug at a higher dosage and he might be more agreeable. But not when he sees this false information in my chart. so a drug that might have been best for me but at a stronger dose is denied and since doctors like to be secretive, I might never know why the drug was denied and have a chance to expalin.
And it bothers me that he may have altered other records or simply not given me the full or 'real' set of records. All I wanted at the time were onlythe office visit records.
Question. Should I ask for other parts of my file to people that work in medical records? If so, what would you suggest? ARE there 'hidden' records-like one set for a aptient and the real set?
And has this happended to anyone else?

In my opinion its a double edge sword....1st let me say you are right to be suspicious, I would be too....Yes I had a Dr do some shady things before, but I just changed Dr.'s for this reason......If you get others involved or make a fuss, you are asking for a negative or in your case a more negative reporting of you and your habits in his file, be it true or not...Keep in mind the next Dr. might think you are a pain and not want to get involved or afraid you will turn on him/her as well.....Like I said find a new Dr. and start from scratch.....Good luck

Thanks for the reply.
I agree tht it would be best to start over but in my experience, youhave to build a relationship with a doctor before he is comfortable enough to prescribe anything that would even be in the same league as my present doctor.
And if you live in chronic pain, I can't wait for maybe a year of Tylenol 3 and so on.
I hadn't thoght about a doctor giving you a negative on your med records. So this goes back to my question: Are their TWO sets of records?

I agree with Cozmo, if your present Dr. is doing ANYTHING for you I wouldn't cause a fuss cause all he has to do is send you a certified letter saying he can't treat you anymore and your gone and you can bet if the next Dr. calls to verify your records you would be lucy if the new guy takes you as a patient and if he does you won't get anything stronger than motrin.
After 15 years of being ingured and contless Doctors I've learned never argue with them and take what they give you, They spend every day of there lives dealing with people who want drugs from them and if they loose a patient theres 10 more waiting to be seen and they take clases to decipher who is truely in need and the seekers I'm NOT saying who is seekng and who is legit I'm just saying these Doctors are hardened to our reality and there not the one's who suffer and not too many of them care. We all have about read Doctors loosing there licenses do to over prescribing and they don't need the grief, so if you don't care for the treatment your getting I would try to find another and when you do you can see him. The one that says that he can't treat you. If you look at some of my old posts, you should see mention of 'The letter'.
That happened about last August.
I had no idea this was coming and I have seen this guy sine 1999. He did mention going to a pain specialist. The guy i see is a GP. But I have been through all the hoops . The pain doc just wanted to give those back shots that don't usually work on most people and are actaully damaging in the long run.
So I told him (again) that I have seen a pain specialist and he dropped the matter.
This letter came on a Thrusday and this was a day that he normally does not see patients. This is a day that he does paperwork and that day, was hosting some kind of seminar at his clinic. I barged in his clinic and waited until he had a free minute since I wanted to know what was going on. With another doctor loking on, he admitted the letter had been a 'mistake'. He mentioned some issues we were having a few months earlier and just supposedly thought to one or more of his staff if he was helping me. This is his version.
Could be his staff becasue I recently found out that his second office manager has left about August. I was friends with his orginal one. The one that is in charge now does not like me but I have done nothing to her. Just one of those types.
He is into anything that can make a buck these days, workers comp, and he breifly had a web sit that listed things I never even knew about but had also tried some like accupunture.
His staff are a-- holes to put it mildly and they are so incompentent, that they ran off a friend that I referred.
Long story short, There was not explanation and legally he violated several areas. The least is I can report him for violation of ethics. He stopped treatment without warning and he did not sign the letter but his office manager did.
I suspect with the second manager leaving, the one that does not like me thought that she could terminate me and I would just go away. This is what I think is the truth. My doctor sure looked sheepish when that other doctor was watching us.
Someone who has a relative as a doctor says that is not legal for an office manager or anyone else other than the doctor to sign the letter.
He is supposed to refer a patient he terminates acording to this other doctor and using common sense.
At an ethics committe, I know would be stacked with his peers but several questions would be asked by me.
1. Why have you prescribed lortab for 4 years and continued that as break though pain and class 2 drugs for primary pain control for over a year and suddenly stopped treatment and did not offer a referral?
2. I am going to be polictially incorrect and say these are addictive drugs and you just don't make someone go cold turkey . People have a way of dying that way. (I would have considered suing over this point for mal practice)
3. If you thought you were so ineffective, why was I a patient for over 5 years? I never stuck you with an unpaid balance and paid when my insurance comapnay was slow at one time.
4. I have never called and asked for a new scrpt like the orginal got stolen in all these years and have gone to bat for my insurance comapny to get payment to him ASAP. I monitor this online and it takes about 1 month. Not bad. I even straightedned out his staff's billing by getting two visits paid for that they miscoded. The woman doing the billing insisted that they sent the correct billing. She was wrong . I gave her the number to my insurance company and about 20 minutes later called ME (a first from his ofice except to cancel an appointment). She had been kind of snotty while I was trying to get things corrected and Miss know it all but was now was 'friendly'.
These are just the points I can think of. I could think of more but another year and a half and I find a doctor more local or I can head where ever since I am retired with no strings except an aging mother who can come with me if she wants.
I am not flaming you on your reply and I thank you and anyone who does reply to a thread of mine.
But I am still angry about how I have done nothing and endured routine 3 hour waits and two times I had the flu so bad that I felt like I was dying. Over two weeks to see him. I could go to the charity ward and get better care 9actaully the hospital is world calss in this area but I won't name it-I do not want the gulity to be identified with a post) Used to, I got in that day or the next at the latest. Now its two weeks if I am lucky and his practice has decreased since I became a patient.
I know that he is having money issues at the business and my insurance is very good and I don't think many new ones are on the horizon. Actually, I am worried he won't be around for that year and a half.
But I posted this also because it is time that we quit begging for what is a RIGHT. These people go to medical school and are screened. How so many A-holes make it into med school confonds me.
Sorry for venting and again, don't take this personally.

Im reposting this I think this will help you with a new DR....Ok I know it will
I personally have never had a problem getting meds, ever....Maybe Im lucky? Or it could just be that Im truly broken....lol
I think If your case is obvious its pretty easy...For those whos cases arent so well documented let me give you the most useful tip in the world for success.
Keep a diary....having a log book with every day using a pain scale of 1-10 and describing symtoms (do not make everyday the same) be realistic....cold days worse, rainy days etc...having a log book with a lot of info....is just as good as a reccomendation...It shows you have a legitamite problem and need, and it covers the Dr.s butt as well...
Try it....it will work....By the way you can find all sorts of diary/daily log sheets for pain on the net. All you have to do is print them out.

Thanks for the idea. I did try that one month and the doctor semed inteested but I got busy and never did it again.
As for documentation, here is what I have
4 MRI's the last done March 2004. All show a progession of disk desease.
I mentioned that I am retired. I worked for the state and they approved early disability retirement.
I got social security disability on the first try (9 months) and the reason it took so long was the backlog of cases I found out.
I was re-eveluated by social security and got a questionnaire called a 'mailer' which is a two page form with 7 questions. Very eay and no doctor's signature required. But I did a LOT of reading on these re-evaluations and how these mailers are coded.
There is a probablity score that you will or will not return to work. This is the most important thing I found out
The higher the score, the more likely you can return to work. I got this mailer in June of 2004 three years after getting my first ss check.
My score was 0138. The highest score is 9999.
This is based on may things and afte a person reaches 50 years old, these mailers will be used every 3 years as opposed to full medical reviews are not usually done.
I think I have a lot going in my case. Add this to what I would have told the ethics commettee.
But your idea is a good one.

I too was accepted my 1st try for disability...Im only 35 and my review is every 5 years....basically means, possible I could get better, but not likely....I too did a lot of research before applying. As far as doccumentation...Ive had 21 sugeries so far...another this month on the 24th and if all is well only 1 last surgery will I need, but its a biggie tho...replacement of my right back joint.
Basically I loaded them down with paper work just like they do us...lol
