Cindy,
How's the plan coming? I hope you're doing well; you situation is very scary. Lots of good advice above, so I won't repeat it. However, there are a couple of concerns not mentioned yet. Although I am not a doctor, I do know a little about dependency and drug interraction. Hydro withdrawl itself doesn't kill, but a poorly designed or executed detox plan can.
If you decide to try ultram (tramadol hcl) for your pain after hydro withdrawl, be sure to do it in consultation with the doctor who is prescribing your antidepressants. Wellbutrin can reduce your seizure threshold, and ultram can induce seizures. Ultram also has some SSRI and norepinepherine reuptake inhibitory effects which may interract with your antidepressants. Also, tramadol dosage may need to be regulated based on the status of your liver. That may require diagnostic testing to determine the extent of liver damage. Finally, if you begin taking ultram while discontinuing benzos, be careful; benzo withrdrawl can also induce seizures. If you are keeping your benzo use low (4 xanax in one day is *not* low, it's huge) and using a long half-life benzo like valium instead of a short half-life med like xanax, you should be able to taper and stop the benzos easily since you are planning on using benzos for a short period of time. However, if you are adding tramadol at the same time you are discontinuing the benzos, I would assume it would be safer to taper from the benzos very slowly, but that's just my applicatoin of logic, not medical opinion. See the Ashton Manual for information on tapering off of benzos: http://www.benzo.org.uk/manual/
Finally, study up on tramadol yourself. It seems to be a good pain med for many people, and most people do not chase euporia by escalating dose to the same extent as with hydro or other opiates. However, if you do escalate dose with tramadol (and some do), you are heading for serious seizure risks. It will also induce physical dependency like any other opiod. Anecdotal reports on tramadol withdrawl vary from hellish in some to mild in others. As usual, results tend to correlate with duration of use and size of dose.
I truly hope you are doing well and on schedule, but if you are not, you better get medical help yesterday. Too many liver toxins, too many respiratory depressants, too much potential for addiction substitution--too much potential to lose it all.
Prayers and well-wishes go out to you on your journey.
