FDA approves new drug for severe pain

Intended for patients who don't benefit from morphine

WASHINGTON - The government approved a drug Tuesday that offers a new way of fighting severe pain — an option for patients who no longer benefit from morphine and other traditional pain medications.

It’s the first in a new class of drugs that selectively blocks the nerve channels responsible for transmitting pain signals. It will be marketed as Prialt and should be available by the end of January.

“When you’ve taken all the kinds of pain medication that there is and you still have pain, that is a very frightening situation,” said Dr. Lars Ekman, president of research and development for the drug’s Ireland-based manufacturer, Elan. “When you have that kind of pain, there is nowhere to go.”

The drug is part of a new class known as N-type calcium channel blockers. It is known chemically as ziconotide.

Morphine is the standard treatment for severe pain from cancer, AIDS, amputations and other significant illnesses, but its effects eventually wear off and the dosage must be increased. At some point, many patients switch from taking medication by mouth or by injection to a microinfusion pump implanted under the skin that delivers drugs directly into the fluid surrounding the spinal cord.

Ekman said about 35,000 to 50,000 Americans have these pumps now. The Food and Drug Administration approved Prialt for patients who are already using these pumps but not getting effective relief from them or who cannot tolerate the available treatments.

This is the first new drug in 20 years to treat pain using such a pump.

Serious side effects
Prialt has been studied in patients with cancer, AIDS and other chronic pain, such as back pain. More than 1,200 patients took part in three clinical trials.

There are side effects, and the FDA was including a “black box” warning — the government’s strongest warning short of a ban. Side effects may include dizziness, drowsiness and altered mental status, with patients confused at times.

Despite the side effects, the drug was approved because there are no other options for these patients and the benefits outweighed the risks, said Dr. Robert Meyer, director of the FDA’s Office of Drug Evaluation II.

“Because this is such an important patient population where they have such pain and they have so few options we felt this drug does offer some real gains,” he said.

Patients with a history of psychoses should not receive it, and all others should be monitored for signs of cognitive impairment, he said.

The idea for the drug came from a snail called the Conus magus that lives in the South Pacific, which paralyzes its victims with venom after capturing them, the company said. Researchers set out learning how to develop a drug based on this venom and eventually copied the amino acid sequence.

Elan would not say how much it plans to charge for the drug.

I just wanted to state that your quote at the bottom about AIDS is how do I say, it's so true and you are a rare and open minded person for having said so. So many blaim those poor victims of AIDS, not knowing the circumstances. I know it's not your problem, and it's not mine. But it is our problem to help these people, to find a cure and to treat them like human beings with all the compassion in the world. Well said, blunt, true, but compassion is so evident in what you said-those of us who are not suffering from AIDS can not possibly know the pain, and too often shame of having such a horrible thing to deal with. Just my ramblings. My admiration to you. Little Kayla

Kayla - I could not agree with you more - AIDS is EVERYONE's problem - and people that do not understand that or are judgemental re people afflicted with this "virus" are ignorant and IMHO need to be sent to a HOSPICE for terminally ill people with AIDS just to see what life is like for some of these poor souls. If you have cancer everyone feels sorry for you - if you are HIV+ all anyone usually wants to know is how you got it - that is - you MUST have been doing something you were not supposed to be doing.

I am VERY active in the fight - and it is a fight - to educate the public re HIV. I also know several HIV+ people who are doing very well thanks to the meds now available in this and the more civilized countries. But in Africa it is a whole "nother" issue - and I take our government to task re the rather usless meds they are trying to dump on the Africans....

I heard on the news this morning that this new drug, Prialt, can only be implanted in the abdomen as a pump at this time. Hopefully they will get some 'pill' action going on this. Sounds like it COULD be promising!

I agree with that, aids is our problem. I hate reading about all the aids orphans in africa and other places. The disease sucks, I don't know anyone with it but just seeing stories on the news or in the paper make me wish we had a cure for it.
I guess as long as we keep ourselves educated about it then thats doing something.
I really don't participate in any of these "walks for aids" or any other diseases but I think its a great cause.

Thanks for all who feel the same way, we can NOT judge these poor people, WHO CARES HOW THEY GOT IT. NOBODY deserves to have this terrible illness. Well, minus murderers and rapists. But society looks at someone with AIDS and goes YUCK! Cancer=sympathy. Not saying we shouldn't give cancer patients sympathy and compassion, but AIDS victims should be treated the same way. I know a lady who worked at a juvenile home for runaways, etc. A girl with AIDS bit her. She did not get AIDS but if she had what would people have thought as soon as they heard, not OH that poor lady was just doing her job. It's even harder for men, people assume things about them right away!!! Say they were a prison guard or something, or just your average bar fight and a man got bit by a person with AIDS, what then?? As a whole, society needs to take a step back and STOP this problem with judgement. I once watched this HBO special, Angels or something like that with AL PACINO, and I HATED IT. Sad but true, it made me depressed and revolted. And I couldn't help it. We are all guilty of judging a book by its cover before we read what is truly inside. Okay getting way too serious here, I am just the kind of person who feels so sorry for the underdog, the wimp in school that always got picked on. Gonna stop now, getting too serious and depressing, just feel so strongly on this issue. HOPING EVERY ONE HAS A HAPPY NEW YEAR and a pain free one at that!!!
