I need records, is that all I say to my doctor? Or is there a better way of explaining this? My doctor is really nice and I have known him for years so I am not embarrased to tell him anything. Any ideas?
P.S. I feel stupid for asking that...

Say you are having a dispute with your insurance and you need your records to confirm some things. Works like a charm !!

If you are going for a checkup thats great, it will be current information. If not, just ask him for copy's of your medical records stating your condition that warrants the meds you want to order. I strongly suggest you DO NOT tell him why you want copy's. You don't need to explain your reasons, you are entitled to them under the law. If you feel more confortable giving an explaination, tell him you just want to keep a current running of all you medical conditions in a file at home. Hope this helps you.

You need no explanation whatsoever. You call and politely ask for a copy of your medical records and you will get them. It's the law. However you may have to pay a fee.

And.....you may just want to go in person to request your records because they will require a signed release from you.

You are right but in my experiences I have called ahead because alot of place require 2 - 5 days to have them ready for you and you can sign the form when you go to pick them up...

Call ahead to make sure they have them ready. You may need to sign a release and pay a small fee, $1 a page or so. Tell him you would like a copy for your own records or insurance purposes.

My old PCP's office had a service that they sent the records to. It was going to cost me 45.00. So, I just told them I needed something to show my specific medical condition and it was a lot less than that, like free since it was one page.

I told them I will be changing insurance companies, which is true, and need proof of my most recent physical. It was faxed within ten minutes.
