YOD Repeat Consult Question

hey guys, sorry, this is seemingly a simple question, but I couldn't find a definite answer anywhere.

I am doing my first repeat consult with YOD. Is the only thing that I need to fax in this time the 2 pages that say "Request for Repeat Consultation Only"? It just isn't really clear whether I need to send updated records, id, etc. I just want to make sure that I don't hold things up by not sending all that i need to send.

In answer to your question, I have always just sent the 2 page reconsult form. No need to refax ID. If I have obtained additional med records since the last consult, I send them just to update my file. For instance, I sent YOD the impression of a MRI I had done since my initial consult. Just be sure to add any discount codes in bold print on top of shipping page because that doesn't carry over from consult to consult.
