My orthopedist is the one who sent me to my Pain Specialist but it was after having a total knee and hip replacement.
I have brittle bone disease, which they discovered post surgery so instead of more usless surgery, he decided they needed to treat my pain.

My general practicioner was who sent me to the pain clinic. This was, of course, after over a decade of specialist appointments including ortho and neuro and chiro and PT. But I would guess the usual course of business is for the primary physician to refer to pain management--especially if you belong to an HMO where ALL referrals must originate from the primary care physician.
I would anticipate a more of a consult from the ortho doc. He will do an exam, read your records/tests and treat as he sees fit based on all the information he has. Some doctors will not prescribe meds if a patient refuses surgery, seeing it, instead, as a refusal to try all non-narcotic treatment modalities.
But this is purely speculation. Only the doctor knows what he/she will do in your specific case. Good luck!

I certainly understand your situation about the diabetes and avoiding injuries/surgeries. The physician should as well, but one never knows WHAT to expect these days.
Definitely address your diabetic concerns with the doctor and stress your desire to have a FUNCTIONAL life again. I know the pain clinic I go to focuses strongly on returning their patients to a productive life and they are very compassionate. If he refuses to help, ASK if he thinks a pain clinic would benefit you. Perhaps he would at least give you a referral if he should choose to not medicate you himself. ESPECIALLY with diabetes, which brings its own set of problems to the proverbial table.
Best of Luck!!!

if you do not choose surgery they will probly sya thank you very much and send you on the way. the arthopedist is for surgical correction of a problem/ if you choose other options there is little they can do for you. they will not want to get into long term pain care. notthere speciality
