medical records--i have questions about it

what, exactly, do i need to ask my doctor to get my past medical records? i have had 3 surguries in the past. i have a refill soon from norco. do they have to release the records to you? how long does it usually take? and how many pages of records is usually needed for norco to substantiate your claim? do the doctors office give you a hard time about it(start asking too many questions)? thanks

This subject has been talked about before (if you do a search you will see). Bottom Line: It is your right to ask and receive your medical records. They may charge you a small fee (around $10). If they do give you a hard time- you just tell them that you know your rights and you want a copy. They don't even need to know your reasons. They will mail them to you or you can pick them up at the office. My doctor mailed them to me with a bill for $8.00. I had about 20 pages, but the amount of pages doesn't matter as long as it documents your condition. Get those records!

Usually they make you fill out a form, or you fax a couple lines about how you request the release of your forms from Dr. XYZ and they can fax or mail, or you pick up.

If you've had surgeries, the surgeon always has a written narrative of what they did and what they encountered. Usually 1-3 pages and describes the condition well.

I just called my doc's office and said I need a copy of the past few appointments. Didn't ask about a fee. She faxed me a release to sign, I faxed it back, and the records were waiting for me at the office free of charge. I think if you don't ask for your whole chart, and just 3-4 of your past visits, you might get around the fee. Hope this helps.

Also whenever I have an mri or ct, I always ask my doc. at the appt. for a copy of the report. That way I always have copies of these on hand.
