Physical Exam Forms

Hi everyone,

Many of you may know that there are OPs out there and listed here that offer up to 5 refills. They want med records and then send out one month of medication and a Request for Physical Exam form for you to take to a doctor and have them complete. This is very interesting to me and I am seriously considering it.

My question is the form. Anyone have this experience out there and how did it go with the form? I just don't know what to say to the doctor. Hi, I'm getting meds long distance would you fill this out for me just doesn't seem like the logical thing to say. I don't want to lie but I don't want to get kicked out of the office either. All I need is an exam. Any suggestions on how to approach the examining doctor? Feel free to PM me and thanks to those who respond.

Bonkers, are you referring to UniversityMD? If so, I have been very interested in them for a long time and would love to get more info on them. If they aren't too complicated, I'd love to start using them when I'm due for another consult.

If anyone has any info, PLEASE post some info on these guys. I'd love to get 5 refills for a $90 consult fee and have the scripts dent to United--the cheapest, best OP pharm there is, period!

That is whom I referring to: University. 5 refills of 120 N is terrific. They send the first Rx out and then the medical exam form. But you must take the form to the doctor and have an exam then return it to the OP before they refill. No big deal really, but what do you tell the exam doctor?--Hi, I'm buying some pain meds over the WWW. I don't think so. The suggestion they said was just tell them you're consulting with a pain specialist but my thinking is if I were the exam doctor I would say, then why doesn't the pain speciliast do the exam. So, I know there are other things to do, like insurance or pre-employment, etc. I don't want to be untruthful, but the fact of the matter is, I have a legitimate problem and I can get 24 or 30 pain meds from any doctor, anytime, but that's not going to help me in the long-run. The OP is going to help me. I would love to hear from people who have gone through this procedure and how they handled it. Perhaps there is a portamedic that can do an exam....now there's an idea.

Hello everyone!

I have used University and found them to be a top notch operation. Dr. L was there for a long time and just this past year, another Dr. took his place. I've heard some people say Dr. R is tough although I've never experienced any problems whatsoever with him. The process to get started is complicated but worth it in the long run.

This is what I did......I found a physician in my yellow pages that does DOT exams and other work related physicals. I set up an appoitment and proceeded to tell this doctor that I was participating in a study related to chronic pain and the treatment of, that as part of this study a physical exam was required that documented my condition, and because the program is based out Florida and in order to meet the guidelines, this exam was to be performed by a local physician. He ask me many, many questions but I was prepared and answered them. That was three years ago and I've been back to that same doctor again since then for an updated physical exam. I again told the doctor the same story and he performed the physical and I left with my form filled out. The best part was it only cost me $40. What a bargain!!

It really is an easy process once you get past the fear of these people. Believe me, they fart and poop just like we do.

Yeah I've thought about this too and found myself thinking along the lines of lovely11: going to a "doc in the box" or some urgent care walk-in place with the forms and telling them I just need a routine visit to get a form signed for some nonthreatening sounding reason.

I must say I really like what lovely11 came up with...hmmmm...;).

You are just lovely. Thanks so much. That sounds perfect and I think that's what I'm going to do. We have facilities around that do pre-employment physicials for $45, and I'm going to do it. I know there is a famous back clinic in Florida also one in San Francisco (which I wrote to once and they never called me, nice). Anyway, thanks for the info. It is very helpful. By the way, what is a DOT exam exactly?

DOT stands for Dept. of Transportation, if that's what you're asking? If you want to know what the exam entails, sorry, I can't help you there. I'm sure one of our DB genius' will know.
