Hello,
I have been using DHC off and on for a chronic knee problem. I only have the pain for 3 or 4 days after doing some sort of impact activity, like basketball. Ibuprophine does OK but the DHC really helped. Since DHC is almost all but unobtainable now, I was looking for a replacement that would be effective. I tried Predeus D.C. (SP?) but it does not do much better than the IB. I was going to try Ultram/Tramdol next but wanted some expert opinions from informed forum users. The DHC not only releaved the pain but had a euphoric benefit. THe peredeus DC does not. Any suggestions about the efficacy of Ultram/Tramadol, and what the best sources.

You really have to try it to know how it works for you. The reason is that it has to be converted by enzymes and different people have different levels of the enzymes.
If you really don't respond to codeine, then tramadol probably won't work for you because the same pathway that converts codeine to morphine makes tramadol an analgesic. But maybe you didn't respond to the Perduretas because their release is gradual and you didn't take a large enough dose.
Anyway, DHC is not codeine, and i don't know if they have the same metabolism. Maybe someone else can look it up.

As the above poster said, tramadol works great for some, for others it doesn't. I have knee and back problems and and sometimes kidney stones, tramadol does zero for any pain. All it does do is help relax me. And I had to take 2 50mg pills for that effect. But still, no pain relief. Even if I take 2 more 3-4 hours later. I didn't try a further increase in dosage because my doc advised against it.
I'd suggest to order a small dosage and see how it does for you. Some here swear by tramadol.
Codeine works sometimes for me. Hydro works the best and darvon would be next. Actually morphine is the best when the doc gives me a shot of it, but...
You just have to try various ones to see which work the best for you. We are all usually different in that respect.

Thanks for the suggestions Trampy and Mikevx. I will do some research on the Darvon to see if that might be an option. Otherwise, I will order a small quantity of the Ultram to judge the effect. Either way, I will try to post the results.
Is there any other alternatives I have not explored, any suggestions?

the tramadol metabolite has 200 times more affinity for mu receptors than tramadol its self. like everyone else said, it must be metabolized well.
i find tramadol to be much superior over the following: Oxycodone, Hydrocodone, Morphine, Codiene

Thanks for the link Tone, that is some good information. I understand what inhibition of reuptake of serotonin is, since I take an SSRI (Lexapro). But what is inhabition of reuptake of norepinephrine, and what effect does that produce?
