      #139665 - 02/20/04 04:44 AM

Hey all,
I just got this huge packet from health plan pharmacy(ephy's old pharmacy) in the mail. BIG yellow envelope! This has all thier info and a bunch of forms to fill out and return. This is a first for me as I have never gotton anything like this before. I wonder how much money they had to sink into this mailing venture of thiers. Anyone else receive anything like this?

Was this mail unsolicited, or did they send it to you as a requirement before they would dispense prescriptions to you? If they have to the extent of sending you an actual form for you to sign and remit, they may have a specific reason for requesting your signature in its direct form.

If you have conducted business with them in the past, but have no current or future business planned with them, I can't imagine any particular advantage to signing anything they may request of you, unless they offer a specific incentive for doing so. In either case, I suggest you read what you are signing very carefully, especially if the forms include a HIPAA consent waiver or other disclosure-related authorizations.

Can you describe the forms they've requested of you?

I received the same thing yesterday. A folder with about 10 papers to fill out and the first line slamming Ephy.com and the other papers asking all kinds of personal questions. If you were ever a customer of Ephy's you will probably get it. Mine was promptly tore up and tossed. It's bad enough when they spam your email but now sending what I consider junk mail to my home,in this giant envelope with their name stamped right on the front, is out of line. I'm sure many who use their work address didn't appreciate it either, Don't contact me,I'll contact you.
