First, let me say that I will never try and get a shipping refund if a package gets messed up by UPS or FedEx. (I can't speak for my boyfriend though...)
Hypothetical question-
If you called an OP to ask for a shipping refund because UPS/FedEx screwed up and the OP got a refund, do you think you would actually get the cost of shipping? For instance, say an OP charges, umm..$35 for a Next Day Air package. Say the true cost the shipment from the pharmacy was $20. Would the OP refund you the $20, since that is what they would get back from the shipper and keep the $15 as a handling fee? Or do you think that the OP would actually refund the whole cost?
Like I said, I don't ever want to do this, as calling OP's for anything other than a consult is just a pain in the arse.

Yes, an honest operation does refund to you on shipping credits. How do I know? It has happened on more than one occasion. They credit back to payment source, i.e., credit card.

Yes, OP's will (most anyway) refund you the money they in turn we're refunded. They most likely wouldn't refund a 50$ shipping charge if in fact it only cost them 20$ to ship it. They, as you mentioned, would refund the 20$ they were credited. If they refunded you 50$ they would be losing money, and that would be silly 
Anyhow,
I would imagine many operations would rather pass the refund off towards a credit on their products. For example 20-50$ off the next consult or medication you purchase. This just makes things easier for everyone if, in fact, you were to continue using the said OP.
I would definitely contact the OP about the refund to at least get a credit towards your next purchase because they, most likely, wouldn't hesitate to keep you happy. Money is money however small

Another thing that OP do soemtimes when a shipping error has occured is give you free or discounted shipping for your next order. I had that happen one time with Woody's along time ago. They put the wrong address and zipcode on a package that was suppopsed to be delievered on Saturday, and I couldn't get it until Monday, after I chased the driver down. It was extremely frustrating. However, Woody's did give me priority overnight shipping for the cost of two day. I thought it should have been free, but I was happy that they tried to resolve it somehow.
