I have not ordered from a OP other than PKW in probably a couple of years as Ultram has been sufficient.I am wondering what all is needed when sending medical records.I had an MRI a couple of weeks ago and have the results in hand ,would that be sufficient? I have a cracked tail bone 2 herniated discs 2 disc bulges and severe pain with sciatica down left leg.Do you think this will be ok to get a pain med prescribed? Thanks much for your help! WREN

I'm going to get a physical. That will certainly do it. For the next year at least.
I would guess that the MRI would be a credible document. I think it would do for your case. I'm just taking a guess from what I have read.

That should be enough! I fax my blood work results that were just done in Oct. and also a 1 page record with everything from my b/p to my back pain and the area the pain is in, ect. That has been good enough for me, so that should be no problem for you.

What you need is two things (which can be accomplished in only one office visit if you tell the doc what you want) - A recent physical (within one year) - to sort of show that you are in ok health as far as blood pressure, etc... And a diagnosis of the medical condition you are seeking relief from. What I did (since I have a long-term condition of my spine, that does not require frequent doctor visits - just have to 'live with it' basically according to my family doc) - What I did was went in, and told him I was going to see either an Orthopedic or a Pain-Specialist, and before I went, I needed a good document of current physical, and re-affirmation/diagnoisis of my spinal condition. I told him before the examination that I was doing this for records that I needed. He was fine with that, although a bit disappointed that I wanted to seek help 'elsewhere'..... Anyway so I got a full, good physical and re-diagnosis (x-rays) of my spine, and this is all documented on a couple pages, and has worked fine for me. Best thing to do if you don't have current medical records, is visit the doc and tell them *up front* that you need a good physical and re-diagnosis of your condition for your records, or as documentation to get in to see a specialist. This worked for me anyway. Good luck! Take it easy and I hope the best for ya....

thanks for all your helpful posts, I suppose I may be able to go ahead and plan an appt. with my MRI results snce that is what I need the meds for anyway. Thanks again !
