NWW, general form... Questions.

Hi,

If any of you are familiar with NWW( hehe, that's quite an understatement), they have a pre-formated form for you to bring to your local doctor (physical examination form), to sign about your condition.

My question is, while it makes perfect sense, I'd like to get some feedback about what to say to the doctor about your reasoning for having them sign and fill out the form?

PM'ing is fine, I just don't know exactly what would be the most feasible explanation for the form.
