greetings folks..lurked for a while but never posted.
What is required to order pain medication for southwest medical group. I just moved to a new city and have no medical records. I of course have no insurance and am timid to go to walk in clinic to get an exam strictly for medication. Do i just ask him to fill out a form? How does this work? Thanks for any suggestions and PM's are welcome.

SWMG is one of THE most strict OP's out there with medical records..... From all I have heard and seen they are on the complete other end of the spectrum from the sites that allow ordering without records. Like, strictest of the strict (as far as records-requiring/consultation services go - which about 90% of the OP's offering pain consultations are now [records-requiring]) - So, if you do not have very good records, SWMG is definitely not a good choice..... Other than that, I really can't offer you any help - OP's offering pain meds without records have pretty much disappeared due to recent enforcement of (already in place) regulations on tele-medical prescribing..... You are gonna have a tough time finding narcotic analgesics without records of your condition/diagnosis. Sorry I don't have better news.

NWW will allow you to download a 'form' which you take to your local physician, basically stating (I think, I have never seen the form because when I used them I did send records) - but from what I hear, this 'form' basically allows your local MD (or you could visit another doctor if you don't want to involve your 'family MD' -) you take the form, and it states that he agrees to consent to your seeking pain-relief from another MD. Like, transferring over your care to another party. Once you get the MD signature, you send it to NWW, and they will accept that in lieu of records. But as far as 'no records' - the best you are going to be able to find is Tramadol.

what kind of records are usually needed for say NWW
the form says a chiropractor could fill it out? is this true? thanks again for all the help.

Hehe, I answered your question before you asked it! I guess I am psychic!! 
NWW DOES require records, but like I said they have a 'different option' than some OP's - where they will allow an MD-consent form to be signed by a local MD and faxed in, if you don't have possession of recent records. This basically gives the 'green light' on you transferring your medical care from the local MD to NWW's MD..... I myself, would prefer to actually send medical records, as I would rather not involve my local MD in the process (except getting my records from him/her) Good luck.

I believe there ARE other OP's that have this 'transfer of medical care' consent form option available, although off the top of my head, I cannot think of them. Maybe someone else could help you out on that? Or, the best option - just go in for a new physical and re-diagnosis of your condition, so that you will have current records! (Most OP's won't accept records more than 365 days old) - That is the easiest way. Especially if you have insurance (for obvious reasons) - I myself do NOT have insurance, but this is still the route I chose, because my most recent x-rays of my spine were 2 years old (I have a chronic pain issue that does not require 'frequent check-up's, unless I want to get surgery done) - It seems like alot of work, but trust me when I tell you, it will pay off in the long run!! Take it easy...

SWMG is strict - but Dr. L is awesome and they do give 3 refills, also recently offering 140 per month if deemed necessary.
Has anyone ever used the NWW med form? I'm curious as to a clinic, chiro, or any doc's reaction. Someone on one of the threads said that they were going to say it was for life insurance purposes?!

They too also offer the physical assessment form that can be printed out. I believe that they also offer 3 refills.

I have used chiro records at every OP I've ever used. The reason being that the chiro REPORT is extremely comprehensive, impressively so. Just make sure that you see a chiro that has more of a medical background than a naturopathic background.
