Can someone tell me if i'm reading this correct as of December 31st you must then download the form and fax it? But at the moment you only need to send medical records and ID ?

From what I gathered in reading that, it seems that a person that doesn't have a current physical done in the last few months, needs to have an updated one by December. I don't think you would need to fax that form or get it filled out, unless you haven't had a physical recently. Maybe I'm completely wrong, but that's what I got from that.

When I used them (late summer), I had to fax over my medical records and ID prior to my phone consultation. But with the way things are rapidly changing concerning OP's that may have changed as well. Good Luck!

They seem to be changing their guidlines in medical practices.. Hopefully the rest won't follow the same route.

Hope Not, winter is here, and I should stock up on the "Tuss" . Bronchitis every year without fail. And with no Uspharmacyrx.com or erxonline.com, this seems to be about the only choice. love---eddie
