Hey,
Besides NWW, aren't there a few OPS where you can download a form directly from their website, take it to a local walk-in clinic, have the doc fill it out for you and then fax it back? I thought I came across a few OP sites a while back, but I can't recall who they were.
And when you take this form to the walk-in clinic, what do you tell the doc? I mean, what if he/she asks you what this is, where it came from, what you need it for, etc? They frown upon the OP world, so do you need to lie? I want to try this, but I am just not sure what to tell them. I hate being looked at the way they look at you when you speak of pain meds & OPS in general! 
Also, is there any chance that some of the other OPS will accept this as "records?" It is (somewhat) proof that a condition exists, technically, but I wonder how far it would stretch in the OP world.
Hopefully some of the "seasoned" DBers can help me here!

The form at NWW's site that you can download makes the statement that you are requesting the release of the information because you are receiving supplementary medical care.
Since doctors are frequently asked to fill out forms for a variety of reasons (work physicals, insurance, sports, etc), you should not have to lie to receive the care you need. Simply request confirmation of your condition with as little detail as necessary.
Even if they do know what the form is for there may not be a problem. Some doctor's want to be able to treat their patients with the medications they need but can't for fear of persecution by overzealous DEA agents. Others don't care if you receive pain relief or not but may find it easier to confirm you have the problem and let someone else deal with it.

So the form serves as "records" or proof of condition for NWW, right? I mean, it says that you can download the form and take it to a local walk-in clinic and once the doc fills it out, you can fax it back and get meds.
But there were a few other sites where you also had this option. Just wondering if any DBers know the sites that I am talking about.

Tri-county wants a form now. See the Tri-county post about what kind of form they want. Hope this helps. Used to be Tri-counties wanted ID and med records before first refill.

They sent me that form a LONG time ago and I never filled it out, still got my meds and I've been using them for about 2 years now. They may be a little more strict nowadays with all the hype, but I sent them my records and a copy of my drivers liscense, so I guess I slipped through the cracks way back when. Good luck, they are IMO one of the best OP's out there,I've never had a problem with double billing or anything of that nature. The only problem I ever had was the death of the DR. whose name was on the bottle, so I had to call them and they had DR. I***** call it in for me(it was my last refill on that consult).That was a couple of consults back, so they seem to be on top of things. Just my experience with this OP,

kewlone4u, were you referring to NWW or Tri-County? Also, does anyone know the URL for Tri-County? Can't find any posts w/the URL listed.
