Help! Trying to figure out if an MRI report will satisfy an OP's request for medical records. Do I also have to include my physician office notes? Any help would be appreciated. Thanks!

It depends on how picky the OP is or what mood they are in that day. The MRI report will only suffice if it shows that something was "wrong". Usually it's best to have a page or two that shows what things you've been prescribed. The more detail the better in most cases.
I am sure they will tell you if the MRI report is enough.

rule of thumb::
there is never too much the more you have the better off you are..a mri is perfect as well as docs notes...you will have no problems

No one knows what adequate records are. It's up to whoever examines them. There simply are no standards. Fax what you have, and spin the wheel.
If possible, you may want to use a company that allows your records to be faxed FIRST, before you pay money up front.
Don't fall for the "specials" claiming X number of refills for a single consultation. You'll very likely pay your cash up front only to realize that in spite of the terms, you've just bought a single, very high priced prescription.
The OP business is like the used car business....full of shams. Caveat Emptor! Gathering information is the best defense against falling vicitim to fraudulent practices. Learn the facts. Ask questions. Be aware of the current scam operations.

There is a standard:
Written diagnosis by a physician, better if on letterhead from the practice/clinic.
OP's that want records would rather have a single page with a distinct written diagnosis and signed by a doctor, than 50 pages of ambiguous documentation.

Sorry, I meant former ERX'er!! Big difference. Based on everything I've read, I'm headed toward YOD or NWW... I would prefer to just send the MRI report (shows herniated disc with nerve impingement) because my doc's name is not listed. He's great, but he thinks a glass of wine and a hot bath are cure alls. I literally just need an occasional/minimal amount of vicodin and xanax. Any thoughts are appreciated....Thanks!
