I just printed out the pages you are supposed to fill out and fax to them along with your records.
Is it really supposed to be 8 pages?

Yep. Fill out the 8 pages, then fax them all in with your records and ID, and they all have to arrive in a single fax attempt. Followups are only 2 pages. Their service is excellent, it's worth getting through the initial questionnaire most definetely.
