Hi!
Do they issue refills on their prescriptions? If so is it 2 or 3? Also, my last visit to the doctor when I requested records they just gave me 1 sheet that described the reason I was there. Is that sufficient? I see that there is a form to download on their site to have the doctors office fill out, has anyone used that with success. PM me if needed-Thanks for all help!

'Superbills' - (the receiept you get upon leaving the doctors' office that details procedures done/reason for visit/sometimes diagnosis) - These are only one-sheet - just a basic checklist of what was done during the visit, and which is signed off on by the M.D -
These superbills in my experience have been sufficient with several OP's over the past couple of years - one nice, succint sheet - Mine showed my complaint, my vitals (blood pressure, height, weight,e tc) - And also what was recomended/prescribed during the visit - This has worked with several OP's in the past since it basically gives all necessary information to prove your complaint/source of pain.
However, I have noticed a trend in the past few months where OP's are getting more strict and requiring x-rays/MRI's, etc in conjunction with the 'superbill' -
Fortunately I tried my luck with several OP's in the last 2 years, sending simply a superbill as my records - so now I am 'on file' with a couple of very good/reliable OP's, and now I do not have to send new records for new consultations. However, who knows if they will soon be asking me to send more records?? If so - I guess I will also have to go on a wild-goose hunt to get copies of x-rays (of my spine), etc ,,etc...
But for the most part - one-pagers *will* work - IF they include your personal info, your vitals, your reason for visit/complaint, your diagnosis, as well as any medications that were prescribed during the visit. Superbills work well because it puts everything needed in one nice succint page.
Like I said though, some OP's are becoming more strict and also requiring x-rays or MRI's now.... (My guess would be because super-bills would be too easily altered or faked by people that are trying to scam the doctors)
I'd say in 80% of cases, this will work though - the two main things they MUST see are your VITAL SIGNS, and your COMPLAINT. It also helps greatly if there is a diagnosis and a list of medications that were tried with you during the visit.

Also, if the OP will accept a superbill - this makes things much easier, because you can simply ask for a copy of your visit sheet (one page) on the way out of the doctor's office, and in my experience, they have been willing to xerox me a copy no charge 100% of the time I have asked for it. What I usually say, when I am walking out to pay for the visit - is that I am considering a pain manageent clinic or chiropractor/specialist in the future and would like copies of all my visit sheets incase the specialist would like to see them. That way, I am not even lying - I am telling the complete truth. Because I am infact wanting copies in order to give to a pain-management doctor. So it works well this way. When you tell them you need the records for a possible future visit to a specialist/orthopedic/or pain clinic - they will usually (every time in my experience) - be very understanding, infact last time I did this the secretary on the way out told me I was 'very smart to keep copies of all my records' - This is one reason I don't understand why so many people say they have a hard time getting their med records. OP's do NOT want a 100 page fax of every x-ray and MRI and visit you have done in your life. They want a nice succint page or few telling them of your major complaints and diagnosis. I have never had ANY trouble getting a xerox of my visit sheet on the way out of the doctor's office door.
