(Oral) Transmucosal delivery medications???

Hey DB'ers, I wanted to pick your brains about Transmucosal delivery pain medications. Among other random craap, I get debilitating migraines and as anyone in pain will concur, sooner is much better than later with pain relief. I can usually take Zofran ODT (a wonderful transmucosal anti nausea med) to stop vomiting and proceed with a hydro or percocet… however, there are times when nothing will stop me from “loosing” my pills into the loo. This means either trying over and over (these pills are precious and I'd prefer not to flush pain relief!) or taking Stadol nasal spray (which I hate immensely, what a godawful medication) or going in for a shot. These options suck.
Which brings me to this question… any insight into other pain medications that are not intended for actual digestion? I know of Actiq (lollipops) but errrr, I don’t think this is a common approach to knocking out a migraine, even in the most dramatic circumstances.

What do you think? I wouldn’t want to ask for something obscene (like Actiq) and sound like a huge psychotic wanker, even if it's not far from the truth.

I think you’ve all effectively caught my drift; any input?

Wow this is the second post of yours I am replying to in less than 5min!! LOL!! Ok well I have CDM(Chronic Daily Migraines) and I know exactly what you mean! I hate the options I have! I usually end up in the ER fighting with the doc to give me the same shot that they have been giving me for the past 2yrs(2mg Dilaudid 75mg Vistoril) and then I can finally get rid of it! But I don't go into that dreadful ER unless my migraine last more than 3days at a 9 or higher(pain scale#)because if I do then they are more reluctant to give the shot! I just wanted to say that I know just how you feel and if you ever wanna chat feel free to PM me!! I will keep checking this post for replies that might be helpful for myself also!!

Wow Kelly, daily migraines, what a horrible menace! At one point I had one for 3 days and I flipped out, cussed at anyone who made a sound or turned on a light in my house, barfed so much I lost 9 pounds in that 3 days and eventually ended up driving myself in the middle of the night (never recommend this, OMG!) to an ER where I promptly collapsed onto their floor when a child came up and screamed about 10 feet away from me. I was seen right away after that episode. I don't even remember what I was given but I was made to take a Cab home for sure! I seem to think I was given Fentanyl, Toradol and Phenergan or some such cocktail, who knows; it worked.
Pardon the massive run on sentence there!

I would love to see some reasonable opiate pain killers for oral transmucosal delivery... I mean that Zofran ODT (sublingualy taken, and not too gross tasting!) is a miracle, I'd love to see something like Demerol in that form. One can dream. Thanks for posting, who'da thunk it, such a friendly place DB hehe

I have migraines too. NOt as often as yours but my neuro would say you have that many headaches because of rebound headaches. He also believes that if you take any medication for a migraine you will get a rebound and cause another migraine possibly. I'm having a very hard time with him understanding that I know my body and my routines and that just because he believes something doesn't make it automatically work on me. I'm about ready to talk to my PCP and give him the ole boot. He walked out on me while I was asking him about a past medication he said should've worked. I think he's got a huge god complex. I just wonder if any of them actually *listen* to us. It's so frustrating. All I want is some assemblance of a normal life. But, he barely gave me 10 Midrin for a month. That's it! I tried that years ago and it didn't work worth a Best if kept off the board. Guess I'm just stupid and he's the all-knowing neurologist that knows my body and migraine routines better than me. Can you just feel the sarcasm??

I wish pain free day/week/month on both of you!

Hi Sweetz!! I feel your pain! I was told the EXACT same thing from one of the neurologists that I seen about 4yrs ago! Well guess what I went off all meds for 1month and took NOTHING,not even an OTC pain reliever, and that was the worst month of my life! The headaches DIDN'T go away! They just hurt worse because I couldn't take anything for them! I mean yes maybe some people to get rebound headaches from their meds but not everyone does!!! And that isn't always the cause of headaches/migraines!! So anyways I just wanted to let you know that and ramble on for a minute lol! Feel free to PM me anytime to chat!!

There is some truth to that “rebound” theory, but mostly I think it’s an enormous crock of manure! Cracker Jack Box MD buttheads, often I imagine them like this ……
