what is the best,strongest pain med to get in the hospital through IV, is it demerol, morphine, dilaudid, or others?

Hi-
I can only say that from my personal experience Morphine was the most potent pain med I received when hospitalized for accidents and various surgeries. But each individual is different and their bodies respond differently to various pain meds. What works for one may not work for another. I do know that if you're hospitalized and the pain meds you are receiveing are not doing the job you can ask your Dr for something different.

Fentanyl would be my vote.
Everyone reacts differently to meds though.
And no matter what you are given...it is the dose that makes the difference. (not to mention tolerance-which will affect your response which would be dose related)

I second that, Summer! I started a thread about this very issue (specifically Demerol) yesterday and everyone has a different idea about it. Give me Demerol over morphine anyday, but I can give you the names of 10 people who would disagree with me.

I've had 11 surgeries in the past 6 years and can tell you from my experience, you must be your own advocate when it comes to pain. If something isn't helping you and you have given it a chance to work, SAY SOMETHING to the nurse. When dealing with severe pain, it is always hard to remember that nothing will make the pain go away completely. Sometimes something that helps you just get to sleep will be more psychologically benficial than something that just "takes the edge off"...especially for people with high opiate tolerances (and those who aren't completely honest about the meds they receive from OP's or other sources than the treating physician because they are afraid of persecution by their docs.)

Years ago I had a serious accident and was in the hospital for 110 days with multiple surgeries. Then I was sent home for three weeks and then brought back for 20 days and additional surgery. During that time I had most every pain med available. Demerol did not agree with me and one of my surgeons said I was moderately allergic to it. They put me on morphine IV and it worked quite well, but they had to keep increasing the dose over time. Finally, they switched me to dilaudid, and I will say without question it was the most effective pain medicine I have ever been given IV and IM. When they sent me home to heal for three weeks pending further surgery, they sent me home with dilaudid tablets (about the size of a BB), and that was the most effective oral pain med I have ever taken.

I will say, however, that several years later when I had another painful medical problem the surgeon asked what pain meds had worked well for me in the past and I told him "dilaudid"--his eyes almost popped out of his head. He said "I was thinking more along the lines of Percocet". So be careful asking for this medicine, even though I still think it is the best pure pain med available.

I personally prefer Demerol I.V. to morphine...but I tend to develop seizures...so that's out...I remember after my first spinal fusion (split my abomen from my pubic bone to my naval, they did) I woke up in recovery..and my old charge nurse was my nurse that night...I was screaming and they had given me all the morphine they could...she said, "I'm gonna get you some valium" and I remember screaming..."It won't work, I't WONT WORK"...but it DID! Man, after 10 mg valium I.V....I slept for 3 hours, like a baby...to this day, we laugh about it...

When I have a kidney stone, I like IV Dilaudid. I have looked into getting the oral form for my fibro, instead of the methadone I take, but I have heard the oral form is not absorbed well in the stomach and is not as effective. Fentynyl does nothing for me, and It seems to be the drug of choice for procedures that require "twilight sleep" like wisdom teeth extraction, abortions or D&C's, and minor surgical procedures where they want you semi-awake. I don't know if its my tolerance or not, but the doctor was using it for the primary source of pain releif during a D&C I had after a miscarriage. I was screaming the entire time!! Same thing with the wisdom teeth, only then I had a hysterical crying breakdown. The nurses said that sometimes Fentynyl causes that reaction in some people. I don't know if It dosen't work becuase of my opioid tolerance or if It just dosen't agree with me. Now if I am going to have surgery, I tell them no Fentynyl.
Morphine has no ceiling level, so you can keep increasing the dose until adequate pain relief has been established. I have only had it once, on PCA after a lithotripsy (where they blast the kidney stones out). But the levels were kept so low, it didn't work well. I think that us chronic painers and opiate users should be honest with our doctors and surgeons about your tolerance. You don't have to tell them where you got the meds, most of the time they don't even ask. It is very painful and trauatic to wake up in the middle of surgery. I always tell the docs in the ER or if Im having a procedure that I have been taking narcoitcs for my fibro and have tolerance. They don't ask much, and usually ask what works. Most of the time, but once in a while you get some jerk who automatically labels you an addict.
