Help me - how to talk to my pain doc.

I am seeing my pain doc for the first time again, after having gone there last year for divereticulitis pains. He helped me out with roxies until the surgery.

No the pain has not all gone away, and in addition, sometimes is stabbing. On top of that, my back has been hurting, and an X ray shows arthritis in my lumbar area. It's painful, but I've already been through all the hoops for both disorders. Physical training, X-rays, bloodtests, colonoscopy. I want to work on my back for a while but I can not assure the pain goes away.
But the upshot is that for now I want pain relief, and my primary doc, after twice writing for percocet, and then once for vicoden, refuses to medicate any further pain. story of everyone's life. But I know I am in pain, and can't barely function, and I take 2400 mg IBU a day.
What is the best approach when talking to the pain doctor this week? I really just want some good meds so I can get on with my life for at least a while. I know I can't do it forever but for now, I am tired of the pain,

Hi,
You are right, most of people in chronic or even acute pain (short term pain and I know I misspelled it) have jumped though the expensive, time consuming 'hoops'.

To your question. I would be honest and with as much ibuphrofen, your damage is going to be ruined at some point.

This is what I would do-assuming if I wanted to keep trying with this joker. I would ask for what worked. If he wouldn't work with you, describe your pain intensity, where it is located, times of day, etc. Then I would ask for at least something that might work even if he won't prescibe what works. Fro example. you might ask for Imitrex if he won't prescribe the vicodone. That is what one doctor did for me. Imitrex is 'supposed' to work like a codeine based pain killer without the narcotic properties. It works for some but when I took it, a pill was over a dollar or two where hydrocodone was about a quarter.

But this doctor sounds closed minded. start searching for another one in the mean time ro you might try a pain specialist.

2400 mg. IBU a day? Ugh...if thats being scripted for long term, be VERY careful & get regular kidney function tests...1200 mg. is the maximum daily dosage for IBU. I suspect long term use would even lower that maximum.

When speaking to the doc, explain to him you "DEGREE" of pain, what I mean from degree, is it mild, moderate, or severe, is it dull, throbbing, and/or sharping? does it shoot over to other orter parts of the body? How would you describe the pain on a scale of 1 thru 10. (you know like use exacting and descriptive terms, provide him with the justification!!), the doctor can't feel your pain, so he can only understand and relate to it subjectively, and that's relayed verbally, also add emotion to your act, when you do this, create a sad and irritiated look on your face, do it from the get-go, the minute you reach the waiting room, stand-up and pace a little bit, take both you hand and place it on your lower back, and attempt a partial stretch as if you are in pain and your actions are an attempt to displace or alleviate it (show a little anger, not at the doc, but at the pain) Believe it or not in some waiting rooms you might be monitored or the receptionist will be observing your actions and might secretly whisper to the doc, "that guy looks likes in he's in alot of pain". Once you're called into the office start you "SIGHS", "MOANS" and "GROANS" Then objectively show him where the pain is emaniciating from, and tell him that motrin (giving it some credibility) helps you with the pain somewhat, but ever since you "DEGREE" has increased, the motrin is has become more and more ineffective. Tell him that "the pain is effecting the quality of your life", "You're at the end of your rope", and "you wish there was a doctor out there that would understand your pain". Say all these things with sad face and somewhat teary eyes. Just remember that his physical examination must jive "objectively" to your degree of pain "subjectively". If he happens to write a script for a narcotic painkiller, after a couple days call his office and thank him profusely and tell him that the med has changed your life and the med greatly reduced your pain. You are now better able to live a comfortable life and add-in that since the the pain is gone, you have started hobbies, like building models, or biking through a woody trail. This will re-inforce in the doc's head that he did the right thing, and future visits, scripts will easier to get and you won't have act like a sad person, all future visits you should act upbeat and cheerful. Until the "Degree" of pain increases once again, then you start the sad part again. Don't worry, he'll trust you and good doc-patient relationship will be established. I have a couple of relationships like that, and the docs are "REELLY KEWL" with me. We even talk small-talk on the telephone and I always include them on Christmas list. Like I throw them a bottle of wine or something. Anyway good luck, on your visit. I'm sure you will do fine, try some role-playing at home before you actually go to the doc's office. Kewl!!!

Check out my post on this same topic. Topic is, " First time going to a pain clinic ". Alot of people helped me out in getting the meds that I needed . It's all about the way you talk to your Doc, and finding one that believes in real pain. Check it out, AFI
