I can come up with a couple, but not ones with hydros, unfortunately. I must've missed it - did you say you don't or do have records??? Medsmadesimple has a form you can download and take to a doc-in-the-box that counts as records. It's very basic, and asks something like: "Did the patient come to you and complain about pain? Thanks. Please sign here". You fax it over and you're good to go.
