You submit a medical profile, put your problem why you need the meds and such, then submit for a consult it should be 85 bucks...cheap, the Dr I had was great very nice and insiteful and helpful got the pain meds needed 100 tabs pus 2 refills, they will be 80 plus ship, they are very fast and the best Op I have came across I did a lot of reshearch and found this gem..now if it does not get ruined....I will email it to you when I can find it I cant find it and I thought it was here. I ddo not have it memorized yet...they say you have to send your ID and any med records but i have not had to as of yet..thye have been great.
