Sometimes the doctor also needs to know that you have taken such meds in the past. I included a photocopy of a label in my records when I was needed a benzodizaepene for sleep and muscle relaxation, as well as copies of the records from my regular pharmacy showing that I had been prescribed a number of different meds for these conditions before and it was sufficient.
By the way, if the botttle says "for sleep" or "for anxiety/pain." that does sort of indicate a dX because the doctor wrote the script that way- affirming the you have insomnia/pain/anxiety. There is not really any harm in trying. Any honest OP will not charge you if they deem your medical records insufficient.
