dixierose, I had no problem signing it as well, since It has been a nightmare finding a Dr to treat me, especialy with a narcotic regimen.
My agreement also states, that I must use the same pharmacy to obtain all of my scripts. I use walgreens, and on two different occasions they were out of my main pain med, and said it was on back order, so I had to go to kroger and get it filled. I did check with my Dr, and he said it was not a problem and understood the circumstance.
