My doctor has this "booklet" hanging in his office and I haven't had to sign anything yet...knock on wood, but I don't think he really cares about these things one way or the other. I honestly think that if he did have a way to look this stuff up (our OP usage) he wouldn't take the time to do it, or even have his secretaries do it. I know him outside of his practice and he is a very laid back guy...a little on the "goofy" side

. He can really frustrate me to no end at times (he refuses to give out narcotics unless someone is DYING!) but overall he is a great guy and I think he is very concerned about people's privacy...that's why I have stayed with him this long.
