Hi. I sympathize with you. I have no special knowledge of your state's laws, or whether your med records are relevant & necessary to the permit you're seeking, but it would seem that the authorities would have a pretty good argument that you waived your right to privacy in those records if you sign advanced permission under oath like that. The alternative, I suppose, would be to challenge the rule that they're travelling under to impose that requirement before you submit your permit application. Hope this is somewhat helpful to you.
