CA requires a triplicate form that is issued to the doc by the state. The form must have the doctor's info preprinted from the state on the script. No more than 30 days supply, no refills, no restriction on quantity, must have patient name address dob on form, one copy to doc, two to the pharmacy (one copy is sent from pharm to state office for transmission to DEA. CII must be locked in a safe or vault (automated dispensing machine OK if locked) and must have alarm on facility when closed.

Regardless of state guidelines, all CII are reported to DEA's database and tracked. The system is designed to watch for individual docs who prescribe too much and people who have several active scripts at the same time. Never heard of an arrest of patient with numerous active scripts unless they used fraud to obtain one or more of them...
