How dangerous it is legally depends on your state's laws. Most states make it illegal to use an alias or to give false information or withhold any relevant information in order to obtain a controlled substance. That could be interpreted to mean that you have an obligation to tell a new doctor what you're already receiving. But it's also kind of vague. Also, every one of these web forms from the OPs asks what medication you're taking. And every time i've seen a new doctor or dentist they always have me fill out a form asking what meds i'm taking. It's standard for any medical practitioner to ask, and it's usually a crime to lie if you're seeking a controlled substance.

Some states have more specific laws about requiring you to *tell* a doctor (even if they don't ask) that you have a controlled substance scrip already from another doctor. If you're in one of those states, be aware of it. Ignorance of the law is no defense.
